step 1
The first thing one should understand is that to make someone really age, one has to do more than just to give a grey hair/beard, and wrinkles.
for this exersize we are going to go a little over the top, and somewhat caricaturish.
As we age, gravity takes over, and things start to sag and go south, jowls droop as does the eyes and nose.
I started out cutting those face parts, and with the warp and liquify tools I distorted them.
No need to get fancy with the cutting out, we will blend them together and erase unwanted parts.

step 2
Do the same for the eyes, but don't go too crazy, since you still have to keep the resemblence of the person, but if that that does not matter, than by all means go nuts.
Just give them a slight droop, and try not to warp the pupils into ovals, keep them round.

step 3
Now place all the parts where they belong, and see what the face is going to look like, and make corrections now.

step 4
For the hands I used the liquify filter, and pulled on the knuckles to look like arthritis was firmly established in this poor patient.

step 5
Here in the background photo, I removed any portion of the face that might stick out from behind the additional layers of the new nose, ears, eyes, etc., and removed some of the hairs on top of his head.
