-Created composite from child in Source File and Stairs.
-Desaturated child and used burn tool to create shadows.
-Used Render Clouds and Perspective transform to create subtle grunge / grit on walls only
-Removed Centipede, desaturated, used warp and free transform, set opacity to multiply and added a gaussian blur
-Isolated Doll Head from source image, desaturate, burn tool to create shadow
-Duplicate Doll Head, set hue / saturation down to complete black, added gaussian blur and set to multiply opacity

This is my first attempt on PSTalent.com

Its nothing special, but it looks good.
