I hope you like this

Basic rounded square shape with a "blue gel with drop shadow" style added. The horizontal lines were created using a gradient overlay then using curves on that design with a custom preset. Finally adding in the Site's logo.

Overall - Layer Styles, and Text Layers, Basically, that's it. Simple :)

no images used..only texts and custom shape tool...gradient overlay is a must!!
