Looks pretty good to me. But instead of blacking out that portion of her left hand for shadow you should have just put a heavy shade on it.

Put lady on the background source (tks to sxc kaptain10 user) and work with burn, dodge and erase tool.

brush tool in the eyes, around the hole to try a realistic hole. after i use curves, channel mixer and h&s adjustment layers
