Select the hand and the camera from the original source.

Then add the selection to the bird and boy image. Then select part of the boy and move into the image part of the camera.- then merge the two layers (the hand and camera and the small picture of the boy). move the entire selction to where it looks best.

edit the hand and camera layer and adjust the saturation to 0 to appear black and white.
